Yes, through continual practice it is cen percent possible.

When you speak a matter, it is obvious forming ideas in your  mother tongue, but that to be done in English within a fraction of second. Your Mind will do it if you are always in English circumstances.

What's the right way to have been in English circumstances?
➡️To read English navals, newspapers, magazines at your interesting area. To watch your MT movies along with subtitles in English.
➡️ To speak wantonly in English with your colleagues despite getting some blames. Such blames are your investment to acquire English.
➡️ The thoughts going on surrounding you might be in your own tongue. Just try them in English in English dialects. I mean like a translation, interpretation.!
➡️ Learn Grammar
Focussing as a separate class, learn Grammar. Parts of speech is not a simple one and for school students as you may think. That is the core area in English ocean.
Each part should be known by you. Just you see many who are good in English, they would be well and good in parts of speech. If you deeply touch every part, you can get fortune to know all about Grammar.

You have to be clear in operating clauses such as noun, adjectival, adverbial.
Connecting various sentences with suitable conjunctions is important in English. Lack of this case shall cause struck and so you may lose your fluency.

Nevertheless going deeply in Grammar, you must learn  important elements as said above.
Your fluency status is depending upon also the load of grammar how much you have carried.

You cannot memorise the words but can catch and keep into your mind. Reading many would bring you words along with structures.
Confirm your interesting area. Take that book, hard copies or web or any nonsense, study completely. You shall get a bunch of new words.
Likely newspapers is the right tool to catch words. Truth be told, words can be imported only through reading, not learning by listing words.
